This is a form which allows an Iowa resident to direct his or her physicians and medical professionals to forego the use of life sustaining procedures and treatments or discontinue life sustaining procedures and treatments in the event of terminal illness in advance of that person’s incapacity and or inability to communicate and make decisions about their medical condition and treatment.
STATE DEFINITION
Under Kansas statutory law: “Any adult person may execute a declaration directing the withholding or withdrawal of life-sustaining procedures in a terminal condition.” KS ST 65-28,103
