Download the Iowa Living Will, also referred to as a declaration, which allows a competent adult in Iowa to make a statement about whether or not they want life-sustaining treatment and also allows the appointment of a trusted representative to make medical decisions in the event of the person’s incapacity.
STATE DEFINITION
“A competent adult may execute a declaration at any time directing that life-sustaining procedures be withheld or withdrawn. The declaration shall be given operative effect only if the declarant’s condition is determined to be terminal and the declarant is not able to make treatment decisions.” IA ST § 144A.3
