Download the Idaho Living Will, also referred to as the advance directive, which is a form that allows a person in Idaho to make a determination in advance as to how they wish to be treated medically in the event they become unable to make decisions for themselves. This is a form that informs physicians and other medical personnel of a person’s medical preferences when they are unable to communicate.
STATE DEFINITION
Under Idaho law, “Directive,” “advance directive” or “health care directive” means a document that substantially meets the requirements of section 39-4510(1), Idaho Code, or is a “Physician Orders for Scope of Treatment” (POST) form or is another document which represents a competent person’s authentic expression of such person’s wishes concerning his or her health care.
