Download the Florida Living Will, also referred to as a “declaration”, which is a form that allows a person in Florida to make decisions about their medical care and whether they want to have life sustaining treatment if they ever become unable to make the decision for themselves.  It also allows the appointment of a health care surrogate who will be authorized to make decisons on a person’s behalf if they cannot do so.

STATE DEFINITION

In Florida, a “Living will” or “declaration” means:
A witnessed document in writing, voluntarily executed by the principal in accordance with s. 765.302; or
A witnessed oral statement made by the principal expressing the principal’s instructions concerning life-prolonging procedures. FL ST § 765.101
