The form allows you to choose which treatments you want your medical professionals to administer and which treatments you would forego, including whether you want to be resusitated or whether you want artificial nutrition and hydration and how you want your life to be sustained.
STATE DEFINITION
In Delaware, an “Advance health-care directive” shall mean an individual instruction or a power of attorney for health care, or both. DE ST TI 16 § 2501
