Download the Connecticut Advance Directive, also referred to as the advance health care directive, which is a form that allows a person in Connecticut to determine how they wish to be treated medically in the event they become unable to make decisions for themselves. This form allows the appointment of a representative and allows a person to decide on organ donation.
STATE DEFINITION
In Connecticut, “Advance health care directive” or “advance directive” means a writing executed in accordance with the provisions of this chapter, including, but not limited to, a living will, or an appointment of health care representative, or both. CT ST § 19a-570
