- In case of an emergency – enter your emergency contact in the lines provided – relationship – your emergency contact’s telephone number
- Have you been convicted, pled guilty or no contest, or forfeited bond or collateral on a felony or misdemeanor? check yes or no
- If your answer is yes, you must read the entire paragraph and then explain the details of your conviction
Step 2 – Section 2 – Availability to Work –
Check the appropriate box regarding the hours you would like to work:
- Indicate in the blocks what hours you will be available to work on each day of the week
If you are hired and this information changes, you must take the responsibility to advise your supervisor
