Step 1- Section 1 – Personal Information –
- Provide the required information as follows:

- What positions are you interested in applying for (enter position into the line)

- Are you under the age of 18 – check yes or no
- Are you able to work legally in the U.S? – Check yes or no
- Have you ever worked for Safeway or an affiliate before? Check yes or no
- If so, when, where
- What position was held
- Reason for leaving
- List the names of any relative who work for Safeway if so:
- Name, Relationship, location
- Have you been convicted of a crime outside of a minor motor vehicle violation in the past 10 years
- If so, provide date, location, nature and disposition of the offense
- Date available to work, total hours per week
- Complete the days of the week and times you would be readily available to work
Step 2 – Section 2 – Education –
- Complete all of the information in the block with regard to education:

Step 3 – Section 3 – Record of U.S.A Military and Reserve Service –
- Provide the following information regarding military service:
- Service Dates From month and year to month and year
- Branch
- Reserve Unit
- Meeting Dates
Step 4 – Section 4 – Professional References –
- Provide Professional or Personal References
- Name, Address, Phone Number and Occupation
Step 5 – Section 5 – Employment History –
- List each job held during the last seven (7) year period with a minimum of at least three (3) employers. Begin with your present or position last held. Also, include any periods of unemployment, military service, and amy volunteer or part-time work experience. If necessary, use an additional sheet to include any other related work history beyond seven (7) years.
- Complete the blocks with all of the above stated information
- Have you ever been fired from a job or given an opportunity to resign? – Check yes or no, if yes, explain the circumstances
Step 6 – Section 6 – Read And Sign-
- Read all of the information provided and check the paragraphs off as you read them by initialing
- Once read, understood and agreed to, provide signature and date, as well, the interviewer must also sign and date the form once going over this information with the candidate
