Download DA Form 969, also known as Top Secret Document Record. This document is designed for intra- office, use only. This form is to be used to provide attached top secret information and names, as well as information regarding all individuals who have or shall receive it.
Step 1 – Begin by placing today’s date in the top right corner of the form (yyyy/mm/dd format) – Continue by completing the following blocks of required information:

- From
- Date Received
- Action Office
- Suspense Date(s)
- Register Or Control Number
Step 2 – Complete the rows of information:
- Control Log Or File Number
- Classification
- Number of Copies
- Description ( type, file, ref., unclassified subject or short title and number of indorsements/incls. )
- Date Of Document
- Originator
Step 3 – Distribution Data – Complete the following rows of required information beginning with today’s date next to the title at the top right of the block:
- Date (yyyy/mm/dd format)
- Copy Number
- Addressee (Print or Type)
Step 4 – Intra-Office Routing Data – complete the following blocks of required information:
- For – (choose “Action”, “Information” or provide a suspense date yyyy/mm/dd format)
- Complete the boxes:
- Check
- To
Step 5 – Names Of All Persons Handling Attached Document Or Who Have Who Have Been Informed Of It’s Content Will Be Listed
- Complete the information accurately listing ALL names and dates:
- Name (Print or Type)
- Date (yyyy/mm/dd format)
- Continued space on page two for additional names and dates.
Step 6 – Regrading Information – Basic Document To Be Reviewed For Downgrading As Reviewed – Complete the following blocks:
- Date For Review
- Classification
- Date of Class
Step 7 – Reserved For Use By Top Secret Control Officer
- Top Secret Control Officer Only, would enter comments and information in this block
