The form is used to measure the success of any monitoring and/or extermination that may be needed to ensure the safety of humans and animals. Ticks carry very serious, sometimes deadly diseases. For this reason, it’s vital that there are ongoing surveillance of the tick population, all species. This survey will collect information regarding numbers of ticks and weather conditions for the most part that would affect the tick population.
How To Fill-In
Step 1 – Section 1 – Tick Survey Data/Tick Drag Data – Answer blocks 1 and 2:

- 2. Collector (full name)
Step 2 – Section 3 – Weather Data – Fill in Section 3 boxes a., b., and c.:
- a. Temperature
- b. Wind Speed
- c. Cloud Cover
Step 3 – Section 4 – Complete required boxes in blocks 4 through 7 as follows:
- 4. Site Number (number of site that is being surveyed)
- 5. Grid
- 6. Time (survey is taken)
- 7. Distance (in paces)
Step 4 – Section 9 – Number – Enter the following information in Section 9 a., b., and c.:
- a. Larvae – (number of)
- b. Nymphs – (number of)
- c. Adults – (number of)
Step 5 – Section 10 :
- Index (enter into the index column)
Step 6 – Section 11- Comments:
- Enter comments and observations that could be important to the survey
Step 7 – Section 12 – Total Number Of Ticks Divided By Dragged Area. – Specimens sent to USAEHA For ID – Place required information in boxes 12,13 and 14:
