Inasmuch as cock roaches are pests that multiply in large numbers quickly and do carry and transmit deadly diseases,they must be monitored site by site. Extermination is necessary to control cock roach eggs and adults, therefore this is a critical action and must be monitored closely. On this form the sites to be monitored will be established and listed.
How To Fill-In
Step 1 – Section 1 – Building Numbers:
- To begin, record all building numbers into section 1 that will be monitored or from where collections will occur
Step 2 – Section 2 – Site Number:
- In the far left column, under “site number” list each site number individually
Step 3 – Section 3 – Location:
Under the column titled “location” you will provide an address to each location from which monitoring and collections will occur.
