Download this New Mexico advance directive form, also known as an advance health-care directive which allows a New Mexico resident to designate a proxy to make health care decisions for them in the event they can no longer make decisions.  It also allows the person to designate certain wishes for certain medical treatment in order to sustain life or withdraw life support in the event of terminal illness.

STATE DEFINITION

Under New Mexico law, advance health-care directive” means an individual instruction or a power of attorney for health care made, in either case, while the individual has capacity. NM ST § 24-7A-1
