Download the Pacific Sunwear (PAcSun) Job Application Form that is available to any person who may have an interest in working with PAcSun. All that needs to be done is to complete the application. Be certain to provide all of the required information. Print the information in ink. You may then turn the application into any Pacific Sunwear location management for further consideration.

How To Write

Step 1 – Section 1 –  Personal Information –

This section will require your personal and contact information as follows:

  • Position for which you prefer to apply
  • Date of Application
  • Name – Last, First, Middle
  • Present Address
  • Phone Number
  • Secondary Phone Number
  •  Email Address
  • Are you over the age of 18? -check yes or no
  • Have you ever filed an application here before?
  • If yes, provide a date
  • Have you ever been employed here before?
  • If yes, provide dates
  • Provide the names of any relatives currently employed with this company – enter name and relationship into the blocks
  • How did you hear of this opportunity?
  • Are you applying for FT/PT etc – check one
  • On what day would you be available to work? – enter date
  • Are you willing to travel if it’s required?- check yes or no
  • Are you able to perform the essential functions of the job for which you are applying with or without reasonable accommodation? – check yes or no
  • If no, enter an explanation in the block provided
  • Have you ever been convicted of a criminal offense (marijuana and minor traffic offenses excluded)- If yes, state the nature of the crime and the disposition(s)
  • Do you have a legal right to work in the United States?- check yes or no- you will be required to provide documentation

Step 2 – Section 2 – Employment History –

In this section you must begin by providing information on your current or most recent past employer.  Provide the following –

  • Company Name
  • City – State
  • Telephone
  • Dates employed – From and To
  • Job Title
  • Supervisor
  • Hourly Rate or Salary- Starting and Ending
  • Does this company have permission to contact this company?
  • Work performed
  • Reason for Leaving
  • Please list and explain all periods of unemployment during the past 5 year period – From and To – Reason for unemployment
  • All past employment inquiries will be set fourth in the same format.

Step 2 – Section 2 – Educational History –

  • Education
  • Name of School and Address
  • Graduated? – check yes or no
  • Number of Years Completed
  • List Diploma or Degree
  • High School
  • College
  • Graduate
  • Other
  • Also list and office, clerical, computer skills or other skills that may be related to the position for which you are applying – enter into the block provided

Step 3 – Section 3 – Availability –

  • List all of the times available (am and pm) that you would be available to work each day of the week in the boxes provided

Step 4 – Section 4 – Professional References –

You must list 3 professional references not related to you, who have knowledge of your work performance-

  • Name
  • Occupation
  • Dates known
  • Company
  • Phone number with area code

Step 4 – Section 4 – Applicant’s Acknowledgement and Signature –

You must read all of the information below prior to providing signature. Once you have taken the time to read and ask any questions you may have to your manager and/or interviewer, if you feel you understand and agree with the terms as set fourth on the application- provide the following:

  • Applicant’s Name Printed
  • Applicant’s Signature
  • Date of Signature by Applicant
