The Alabama health care power of attorney form, under Statute 26-1-2(g) and often referred to as the Advanced Directives for Health Care (Living Will and Health Care Proxy), allows an individual to hand over the rights to their medical treatment in the chance they cannot due so themselves.

This forms includes a living will which can also give the patient the right to set out their own health parameters in the chance they do not have someone they would like to appoint as their agent. The agent, if they have durable (financial) powers as well, may reimburse themselves for any necessary meals, lodging, and travel expenses. This appointment is not to be a paid position.
