Download this Virginia Advance Directive form, also known as, Virginia Advance Medical Directive, in order to designate a trusted relative or friend to make medical decisions on your behalf in the event you are no longer able to.  It also provides options to choose certain treatments in the event that you are incapacitated.

STATE DEFINITION

Under Virginia law, “Advance directive” means (i) a witnessed written document, voluntarily executed by the declarant in accordance with the requirements of § 54.1-2983 or (ii) a witnessed oral statement, made by the declarant subsequent to the time he is diagnosed as suffering from a terminal condition and in accordance with the provisions of § 54.1-2983. VA ST § 54.1-2982
