Download this New Hampshire advance directive form, which allows a person in New Hampshire to decide in advance how they want life-sustaining treatment to be administered in the event of their incapacity.  It also allows the person to choose a trusted relative or friend to make decisions for them if they are unable to communicate.

STATE DEFINITION

Under New Hampshire law, “Advance directive” means a directive allowing a person to give directions about future medical care or to designate another person to make medical decisions if he or she should lose the capacity to make health care decisions. The term “advance directives” shall include living wills and durable powers of attorney for health care. NH ST § 137-J:2
