Download the Hawaii Advance Directive, also referred to as the advance health-care directive, which is a form that allows a person in Hawaii to specify to their health care professionals, treatment preferences, such as pain remediation and hospice care, in advance, in the event a person is close to death or becomes incapacitated.  This form also allows a person to choose a person they trust to make and carry out health care decisions for them if they are unable to.

STATE DEFINITION

Under Hawaii statutes, “Advance health-care directive” means an individual instruction or a power of attorney for health care. HI ST § 327E-2
