Download the Checkers Job Application Form that is made available to complete and provide to Checker’s Management for employment consideration, with one of the many locations. Simply complete this form entirely to avoid delays. The company requires that all, except for applicant’s signature be printed in ink. Also be advised that this company will test for all drugs and alcohol, prior to and for the duration of employment.

How To Write

Step 1 – Section 1 – Personal Information –

Begin by placing the date at the top right of the application, then continue with the rest of the application –

  • How long have you resided at this address?- enter answer in line provided
  • Social Security Number
  • Telephone
  • Cell Phone
  • Position Applied For- check one
  • If choosing part time- specify the hours of availability on each day of the week – enter on the lines by each day
  • If you have additional time restrictions, explain this on the line provided

Step 2 – Section 2- Education –

Provide the information requested with regard to education you’ve acquired –

  • Type of School
  • Name of School
  • Location with complete mailing address
  • Number of Years Completed
  • Major and/or degree
  • High School
  • College
  • Business or Trade School
  • Professional School

Step 3 – Section 3 – Legal –

Have you ever been convicted of a crime? – check yes or no

  • If yes, explain number of conviction(s), nature of offense(s) leading to conviction(s), how recently such offense(s) was/were committed, sentence(s) imposed, and type(s) of rehabilitation imposed by the courts. – Enter explanation on the lines below, if additional pages are needed, attach them to your application
  • Do you have a drive’s license? – check yes or no
  • What is your means of transportation for getting to work? – enter answer on the line provided
  • Driver’s license number – enter on the line provided
  • State where your driver’s license has been issued- enter on the line provided
  • Check the box, next to the type of license(s) you hold

  • Have you had any accidents in the past 3 years? – check one

  • Any moving violations in the past 3 years? – check yes or no

  • Do you have vehicle insurance?- check yes or no
  • Certificate of Insurance from your insurance company showing minimum liability amounts of 100,000/300,000/100,000 must be shown upon hiring. Are you able to provide this certificate with these liability limits?- check yes or no

Step 4 – Section 4 – References –

You will need to list two personal references other than relatives or previous employers –

Step 5 – Section 5 – Military –

Step 6 – Section 6 – Work Experience –

You are required to list your work experience for the past five years. Beginning with your most recent job held. If you were self-employed, give firm name. Feel free to attach additional sheets if necessary. –

  • You are required to list the jobs that you’ve held, duties performed, skills learned, advancements and/or promotions earned while you worked with this company.

  • If not, please enter the name of the person who did on the line provided
  • Please provide a list of at least 3 work references along with name & phone number, who would be willing to attest to your work ethic and moral character.

Step 7 – Section 7 – Application Form Waiver –

  • In this section, you are required to read carefully the four paragraphs one by one. With each, at the end of the paragraph, if you understand and agree, you my initial and date it.
  • At the bottom of the form, if you were able to understand and agree to all four paragraphs your signature and date that the application was signed by you must also be dated
