Download DA Form 2162-8-1, also known as the NCOER Counseling And Support Form. This form is a United States Army form that is required as an annual NCO evaluation report of a non-commissioned United States Army officer during a counseling session. This form must be completed by a supervising officer only. This form is now referred to as “counseling and support form” as opposed to it’s previous consideration as an “evaluation report.”

How To Write

Step 1 – Part I – Administrative Data – This portion of the form is the basic information needed to evaluate the officer. All raters must sign off with regard to the evaluation report before it’s sent out.Complete as follows:

  • a. Name of Rater (last, first, middle initial)
  • b. Social Security Number
  • c. Rank
  •  Date of Rank (mm/dd/yyyy format)
  • e. PMOSC
  • f. Unit
  • g. Status Code
  • h. Rated NCO’s Email Address (.gov or .mil)
  • m. UIC
  • n. CMD Code
  • o. PSB Code

Step 2 – Part II – Authentication – Carefully complete as follows moving across the columns:

  • a. Name of Rater (last, first, middle initial)

Step 3 – Part III – Duty Description (Rater) – This area is the actual evaluation information for the NCO’s report – Complete as follows:

  • a. Principal Duty Title
  • b. Duty MOSC
  • c. Duties and Scope – (include appropriately, people, equipment, facilities and dollars)
  • d. Areas of Special Emphasis – (areas in which the NCO excels etc)
  • e. Appointed Duties
  • f. Physical Fitness and Military Bearing (inside of this block include APFT, APFT Date ( mm/dd/yyyy ) and Height/Weight as required)

Step 4 – Part IV – Army Values/Attributes/Skills/Actions(Rater)Values/Attributes/Skills/Actions(Rater) – ie: Loyalty, Duty, Selfless Service, Respect/EO/EEO. Integrity, Honor, Personal – The evaluating officer will decide whether the NCO has exceeded, met or failed to meet standards of the Army’s core values. -Complete as follows:

  • Address Task/Action (place all comments in the block provided)
  • Performance Summary- (provide information with regard to performance in the block provided)

Step 5 – Continued from page 3 – The rater must report specifically on five areas of the NCO’s abilities. These areas would include – competence, physical fitness, leadership, training as well as accountability and responsibility. The Evaluater must state if the NCO, exceeds, meets, needs some improvement or needs much more improvement and provide comments accordingly. Complete as follows:

  • Rated NCO’s Name (last, first, middle initial)
  • Social Security Number
  • b. Competence – Fill the circles that apply to this NCO
  • Task/Action – Describe
  • Performance Summary – Summarize

c. Physical Fitness and Military Bearing:

  • Task/Action- Describe
  • Performance Summary – Summarize

Step 6 – Once the evaluation is complete it’s ready for submission
